Responsibilities and Duties Complete structured medication reviews Process specialist treatment letters Process hospital discharge letters Act as a contact in practice for all medication-related queries. Medication Reviews Manage long term conditions Contributing to the improvement of health outcome in long-term conditions Providing specialist knowledge and advise on medication matter Completing discharge letters Supporting Practice achievement of key impact and investment funding QoF targets as required. Carrying out clinical audits.
