Summary
Main duties of the role: Team Development & Education Lead the structured upskilling of clinicians, supporting the transition from minor ailment management to acute and frailty-focused care, including the following: Care home resident care Palliative care management Chronic disease management Frailty management Proactive care/Personalised care plans Falls management Prescribing across the team Deliver teaching, mentorship, and clinical supervision tailored to staff development needs. Foster a learning culture that encourages reflective practice and continuous improvement. Service Integration & System Leadership Collaborate with wider primary and community services to embed integrated frailty pathways Through the work of integrated neighbourhood teams, design of new models of care aligned with ICS and population health priorities. Actively participate in multidisciplinary team (MDT) meetings, safeguarding discussions and anticipatory care planning. Quality Improvement & Governance Support clinical governance activities, including audits, incident review, and protocol development. Promote evidence-based practice and continuous service evaluation. Ensure adherence to national and local guidance, including NICE and frailty assessment tools.
