Crynodeb
Carry out holistic assessment of patients specialist palliative care needs and develop an initial care plan to address any urgent needs. Communicate and liaise with the patients usual care team including their GP, District Nurses, existing specialist palliative care professionals, out of hours services, reporting on the condition and needs of the patients and providing expert advice and support to address their urgent needs. Establish excellent working relationships with internal and external peers and colleagues to foster positive and collaborative working. Offer support, guidance, and education to less experienced members of the rapid response team, and other professional visitors to the team to encourage individual and team learning and development in specialist palliative care.
