Crynodeb
To participate in clinical pharmacy services by screening prescriptions, counselling patients, advising healthcare professionals, and contributing to safe, effective, and evidence-based prescribing practices. To support discharge planning and medication continuity by undertaking drug histories, transcribing discharge medication, liaising with GPs and community pharmacists, and facilitating early patient discharge through counselling and appropriate supply. To contribute to clinical governance and risk management through documentation of clinical interventions, participation in adverse drug reporting, attendance at ward rounds, and involvement in audit and therapeutic drug monitoring. To dispense, distribute, and supervise the supply of medications, including controlled drugs and clinical trial items, in accordance with Standard Operating Procedures and ensure accuracy and timeliness in prescription handling. To manage the use of pharmacy systems and automated dispensing technology, tracking prescriptions and supporting the accurate supply and documentation of medications across hospital wards and departments. To ensure compliance with statutory and local medicines regulations, including controlled drug checks, formulary control, and storage and administration protocols within clinical areas.
