Summary


Band 3 Community Cardiac Rehabilitation Worker

An exciting opportunity has arisen for a Community Cardiac Rehabilitation worker to join our motivated, enthusiastic Community Cardiology team working in the home exercise programme.

The post involves visiting patients in their own homes to facilitate individualised exercise programmes for patients post Myocardial Infarction, PCI, cardiac surgery and for patients with heart failure. An exercise programme is prescribed by our Band 7 physiotherapist; the successful candidate will support, motivate and deliver this programme.

Clinical supervision and support will be provided to the post holder.

Applicants must be a car driver with full use of their own car and will require business insurance.

• To assist the Registered Nurses and Physiotherapists and work autonomously in providing high quality patient care within the Community Setting.
• Checking baseline observations, signs and symptoms prior to delivering the prescribed, individualised exercise plan and set goals.
• The job includes covering cardiac rehabilitation phase 3 group work at four Centres across the City alongside a Cardiology Specialist Nurse as required.
• Car Driver is essential.
• Ongoing support, clinical supervision and the relevant training will be provided

• To assist the Registered Nurses or other registered health care practitioners in providing high quality patient care within a community setting and in patients own homes
• Maintain patients privacy and dignity at all times
• Be an effective part of the multi-disciplinary team contributing to the departmental objectives
• Continue to develop and maintain competencies in nursing tasks to maintain high quality safe care
• To work with a physiotherapist and nursing staff to deliver exercise programmes in the patients home and community venues
• To provide cover for the cardiac rehabilitation centre group classes in the absence of the band 4 fitness instructor.
