Summary
The post holder will provide full patient care i.e. independently investigate/diagnose patients referred to the service and subsequently inform patients of diagnosis, request further investigations and liaise with multi-disciplinary members. The post holder will be in receipt of a formal non-medical independent prescribing qualification in order to be responsible for prescribing and reviewing patients drug treatments. The post holder will be recognized as an acknowledged expert in the management of patients with chronic disease and maintain clinical excellence in this role through the maintenance of a live portfolio. The post holder will undertake research and audit which will directly improve and impact on patient care. The post holder will provide supervision, teaching and development opportunities for nursing, medical and other related professionals.
