We currently have an opportunity for a Registered Mental Health Nurse post within our North Manchester Specialist Community Mental Health Teams based in our exciting new and modern base at the Central Park Hub.
Mental health services in Manchester are commencing their journey around Community Transformation. Working towards a new community mental health framework vision for a place-based community mental health model.
As part of this new model, we are looking for a skilled and enthusiastic individual with strong clinical skills who is motivated to support the development of our Specialist CMHTs; with the focus on monitoring and improving the physical health and wellbeing of our patients, using the PHIT assessment tool, as well as safe and effective administration and delivery of medication interventions.
The successful candidate will be an experienced RMN who will play a key role in the development of a new Physical Health and Medication Management Hub across clinics in our North Manchester SCMHTs.

Working in a dedicated pathway within a Specialist Community Mental Health Team, the post holder will deliver on all the aspects of physical health care and medication management which includes managing depot clinics. The post holder will be a qualified Registered Mental Health Nurse.
The post holder will work closely with the Senior Practitioners for Nursing, Advanced Clinical Practitioners, Medical staff and Care Coordinators and the wider MDT to deliver high quality, safe patient care. The post holder will always work to promote parity of esteem.
They will be responsible for the Infection Prevention and control of the physical health clinics. The post holder will demonstrate an attitude which respects and values service users’ and their Carers. The post holder will hold effective communication skills and work closely with a wide network of services such as GP’s, other mental health services and third sector providers.
This will respect individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.

To demonstrate high-level communication skills, providing and receiving complex, sensitive information relating to care plans, support pathways and physical health interventions
To utilise highly developed interpersonal skills including specific skills to facilitate effective communication when there are barriers to understanding e.g. sensory or cognitive impairment or in a hostile, antagonistic or highly emotive situation.
To establish and maintain robust communication networks with clients, carers, and other health care workers and numerous external agencies.
To establish and maintain good working relationships with GPs, GP commissioner’s and relevant consortia.
To promote awareness of the role of CMHT within local service area/team, GP practices and the wider community.
To communicate effectively with the wider the CMHT and escalate concerns where necessary.
Be proficient in completing accurate clinical records and clinical letters.
Be able to provide physical health information in manner that is understandable to patients with complex needs.
Be a full member of the multi-disciplinary team (MDT).
Effectively manage busy depot and physical health clinics across the North CMHT geographical footprint.
Be able to plan and deliver home administration of medication and physical health assessments.
Undertake a full physical health assessment which includes the suite of physical observations, phlebotomy and ECG’s.
Be competent in screening haematological and biochemistry bloods and be able to promptly escalate concerns to the appropriate clinician.
Undertake formal mental health reviews and complete a full mental state examination where needed.
Be able to administer standardised tools on the assessment of psychotropic medications such as the LUNASRS and GASS.
Assess, care-plan, provide interventions and evaluate and record community mental health and social care delivered to clients.
To promote engagement of our service users in national health promotion strategies such as cancer screening, optician services and dental reviews.
To promote positive mental health and physical care to clients in all situations.
To encourage and support clients to collaborate in their treatment programme and recovery.
To encourage self-help as much as possible, promoting the maintenance of skills and independence in clients.
To support health promotion and act in a consultative role/advisor to other community services/agencies and personnel in relation to individual clients and their mental health issues.
To take the clinical lead for the service within your area of specialism and provide advice, guidance and support to colleagues and other professionals.
To undertake and record comprehensive risk assessments where needed, devise and implement risk management plans for clients at risk of vulnerability, self harm or risk of physical, sexual or emotional harm to others, using the STAR V 2 Risk Assessment Tool.
To engage carers in the promotion of physical health and medication concordance where appropriate.
To take a lead role in service developments within your area of specialism
Ensure that service users and carers receive clear and accurate information on:
• How to access the service around the clock
• Confidentiality
• Access to Records
• Their own Assessments and Care Plans
• Mental Health conditions and medication
• Service Publicity, including what they can expect from the service
• Community Resources
• Complaints and compliments procedures
• How to access Advocacy Services
Promote a service philosophy based on a balanced model of care, recovery and personalisation.
Support the regular audit of standards that pertain to medication management and physical health and support in the improvements of these standards.
Establish quality standards relating to the service user and carer experience.
Maintain quality assurance systems to monitor and audit these standards.
To participate in complaint investigations when required.
To participate in Serious Untoward Incident Reviews when required.
Participate in service governance initiatives.
To ensure that all Quality Standards are maintained.
Contribute to the writing of shared guidelines/protocols of care.
