Location: Sudbury, Suffolk
Salary: £28,000 per annum (rising to £30,000 upon successful completion of probation)
⏰ Hours: Full-time, 40 hours per week (Days)
On-Call Responsibility Included
Driving Licence Required

This is an exciting opportunity for someone ready to help shape a growing service while delivering exceptional, person-centred care to clients across the Sudbury area.

About the Role:
As a Care Coordinator, you’ll be a key part of the care team, working both in the community and from the office (as the branch develops). You will be the first point of contact for both clients and carers – a hands-on and supportive presence in the lives of those we care for.
You’ll work closely with the office team and local health services to help make sure care is well organised, safe, and kind.

Key Responsibilities:
• Develop and regularly review person-centred care plans
• Support and shadow new carers; conduct competency checks
• Provide hands-on care in the community as needed
• Oversee staff rotas and ensure timely, clear communication
• Support with medication training and management
• Conduct service user visits to review and adapt care plans
• Maintain high standards of dignity, privacy, and safety
• Participate in ongoing training and professional development
• Take part in on-call duties and emergency response

✅ What We’re Looking For:
• At least 2 years’ experience working in care with vulnerable individuals
• A full UK driving licence and access to a vehicle
• A commitment to person-centred care and upholding individual rights
• Strong time management and decision-making skills
• Willingness to undertake NVQs or other professional training
• A proactive, team-focused attitude and clear communication skills
• Knowledge of Health & Safety and safeguarding procedures
