Summary
Conduct medication reviews (face-to-face, telephone, and via digital platforms) for patients with polypharmacy, long-term conditions, or complex needs. Acute and minor aliment clinics within the candidates competency framework Perform physical examinations, interpret diagnostic tests, and prescribe medications as necessary Deal with incoming hospital discharges and third party letters Support repeat prescription processes and medicine reconciliation post-discharge. Educate patients and their families about health conditions, treatment options, and preventive measures. Participate in quality improvement initiatives and contribute to the development of clinical guidelines. Stay current with advancements in medical practices and participate in ongoing professional development. Promote antimicrobial stewardship and best practice prescribing The list is not exhaustive.
