An exciting opportunity has arisen to be part of a community mental health rehab project. The South London and Maudsley NHS Foundation Trust (SLaM) Integrated
Community Rehabilitation Team provides specialist enhanced rehabilitation support to adults typically aged 18-65 diagnosed with severe and
enduring mental health needs who would benefit from an intensive rehabilitation and recovery-focused approach which will best be met by an integrated clinical and voluntary sector shared model of care and support within an accommodation-based service.
The accommodation and support staff are provided by Penrose in the form of two 7 bedded properties in the London Borough of Lewisham, less
than 10 minutes walk from each-other. SLaM provides dedicated medical, occupational therapy, psychology and co-occurring mental health,
alcohol and drugs practitioner input and the posts will sit within a team within the established Psychological Medicine and Older Adult (PMOA
directorate) . Although based in the borough of Lewisham, the service will also be open to service users from Southwark and Croydon boroughs
served by SLaM.
The successful candidate will therefore have experience of working within a mental health environment, have a strong commitment to improving
the recovery outcomes of individuals, and enjoy working with stakeholders to support service transformation and innovation.

· To manage a caseload of clients with complex needs, using evidence based /client centred principles to assess, plan, implement and evaluate interventions.
To offer OT intervention for patients living at the SLAM Rehab Community Provision, being responsible for completing standardised assessments / manualised interventions (including groups) as agreed by senior professional lead

· To apply a high level of understanding of the effects of disability on occupational performance and provide training and advice on lifestyles changes and adaptations to the client’s social and physical environment.
· To promote equality of outcome for service users that reflect their mental health needs and those of ethnicity, gender, culture physical health and age.
· To promote social inclusion and physical and mental well-being using evidence based practice. This is underpinned by the principles of hope and recovery, health promotion and relapse prevention.
· To independently manage a clinical caseload of service user with complex mental health needs, liaising closely with the care co-ordinator and/or vocational lead where relevant.
· To identify occupational therapy goals as part of the overall care plan, using specialist mental health and occupational therapy assessments, including MOHO tools.
· To exercise clinical professional judgement in complex clinical cases and difficult clinical situation, referring to senior staff when necessary.
