Summary
We are recruiting a Cancer Clinical Care Coordinator to work within the Breast Nurse Specialist team who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.
As part of a 1-year Pilot, the Cancer Care Coordinator will work alongside the Clinical Nurse Specialists to set up a new nurse led service and ensure a safe and efficient service for patients. They will contribute towards the provision of a high standard of evidence-based care, supporting the management and supervision of patients throughout their treatment pathway.

Provide a seamless service to patients with proximal supervision from the tumour specific Clinical Nurse Specialist (CNS).
Communicate effectively within the multidisciplinary team and other departments to ensure that patient care is well planned and coordinated.
Have contact face to face and telephone communications with patients, carers and health care professionals.
Contributes to the improvement of services.
Assist in the delivery of our personalised care model and work with patients in completing their holistic needs assessments and care plans.
It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies to be determined within the clinical team with the focus on patient needs.
