Summary
Main Duties and Responsibilities Clinical Responsibilities Provide timely and effective consultations with patients in person and via telephone. Manage a wide range of clinical presentations, including acute illnesses, chronic diseases, and preventive health issues. Triaging and conducting home visits where clinically appropriate for housebound patients. Provide ongoing care for patients with long-term conditions, including care planning and regular reviews. Prescribe medications safely in line with current guidance and the local formulary. Review and action pathology results, correspondence, and medication requests efficiently and accurately. Screen patients for disease risk factors and early signs of illness. Promote health education and preventative strategies to support patient wellbeing. Refer patients to secondary or specialist care where appropriate and ensure continuity of care. Maintain accurate, comprehensive, and contemporaneous patient records using the clinical IT system. Administrative Duties Participate in the daily clinical workload including document management, prescription requests, referrals, and results processing. Ensure follow-up of patients as clinically indicated and arrange appropriate reviews. Contribute to the development and review of practice protocols and clinical guidelines. Work in line with CQC standards and participate in audit and quality improvement activities. Attend and contribute to practice meetings, clinical governance, and multidisciplinary team discussions. Professional Responsibilities Maintain professional registration and comply with GMC regulations and revalidation requirements. Participate in annual appraisal and develop a Personal Development Plan (PDP). Keep up to date with evidence-based practice, NICE guidance, and local pathways. Engage in clinical teaching of medical students and trainees if applicable. Foster good working relationships with colleagues across the Primary Care Network and wider healthcare system. Maintain confidentiality and uphold information governance principles at all times. Practice-Wide Responsibilities Support the practices commitment to equality, diversity, and inclusion. Contribute to the implementation of practice policies, procedures, and service developments.
