Are you a dedicated adult nurse seeking to support patients to stay at home? We’re looking for an experienced clinician to join our team in Oldham, working full-time (36 hours/week) over 7 days, with 12-hour shifts covering days and nights.

About the Role:
You will deliver high-quality rehabilitation and support for patients with complex, chronic conditions and multiple comorbidities. Your expertise will help patients achieve their goals by providing specialist assessments, supporting early discharges, and working with a multidisciplinary team.

Key Responsibilities:
• Lead assessments and treatment planning
• Support patients to remain at home safely or return home
• Work across community urgent care, Discharge to Assess, A&E, and rehab services
• Collaborate with social care and healthcare professionals to facilitate timely discharges

What We’re Looking For:
• NMC registered adult nurse with a degree or diploma
• Experience in Discharge to Assess, Community Urgent Care, A&E, or rehab
• Excellent communication and organisational skills
• Ability to work independently and adapt to service changes
• Flexible team player
