Summary

At Pobl Group, this role is best known as Project Worker. Our Supported accommodation service, Albert Street, in Newport is looking for a Complex Needs Project Worker to join their team on a full-time (37 hrs), permanent basis.

Albert Street provides supported accommodation for individuals over 18, who are homeless and with co-occurring mental health/ substance misuse issues. You could be working with these incredible individuals to teach them tenancy management skills and engage them with external services to really make a difference to their lives.

Albert Street provides person centred and tailored support to individuals accessing the service. We deliver support that meets the needs of the individuals, and which provides opportunity for them to achieve their goals and aspirations.

Day to Day Duties:

As a Complex Needs Project Worker you will support the Project Workers with facilitating person cantered support plans and work extensively with a service users to promote mental health recovery and seek treatment and recovery for substance misuse.

Your day will consist of:

• Be responsible for drawing up the Risk Assessment and putting a Risk Management Plan in place where necessary, in order that risk is managed safely within the service, and have overall responsibility for maintaining and updating these, on an ongoing basis.

• To undertake an assessment of client’s needs and risks, have overall responsibility for a case load of complex needs clients within the service.

• Be responsible for meeting with clients to review support plans, be responsible for maintaining and updating these, on an ongoing basis, provide support to those referred regarding the purpose of the scheme and their responsibilities whilst living there.

• Provide support to understand their responsibilities in line with the Licence/Tenancy Agreement

• Provide support for those staying at the scheme with a view to assisting clients to move onto independent accommodation suitable to their needs and wishes.

• Liaise and work closely with other agencies and professionals with regard to clients’ support needs, e.g. multi agency teams, Social Services etc

• To lead each shift ensuring all necessary tasks are completed and direct the work of the Scheme Support Officers

• Liaise with Social Services, Benefit Agency, Housing Department, Probation Service, and other relevant agencies on matters affecting prospective and current clients of the scheme.

• Show clients around the scheme, explaining to them how the project is run and the layout of the building. Explain House rules, Health & Safety requirements and introduce to other clients.

• Advise on re-housing options and make referrals to other agencies in relation to temporary and permanent accommodation provision, advise and support clients with Housing benefit and welfare benefits where appropriate and ensure that clients know where to claim and that the right type and amount of benefit is claimed.

• Maintain clients' personal files and update records of their progress in the scheme.

• Assist with the supervision of the premises, including day to day running of the scheme.

• Actively promote a Health & Safety culture and to carry out all role specific responsibilities.

• Support all clients to ensure that they pay their rent on time and in full, this may include handling small amounts of cash when necessary, ensuring you follow the cash handling procedure.

• Attend Team meetings and training as required.

• Keep up-to-date with Pobl policies and procedures, ensure that Pobl's Equality and Diversity policy, Professional boundaries and Health and Safety policy are fully adhered to.

• To carry out other reasonable duties as required by the Manager.

• To promote a Psychologically Informed Environment.

Our motivation is to ensure we equip these individuals with the knowledge and tools they need to live independently as well as re-engage them into the appropriate health services. This is a challenging role and you will be faced with complex behaviours and needs, including failing health, complex physical health, and substance misuse issues. You will work with a range of different personalities and behaviours so you will need to be able to adapt your approach to engage with a diverse cliental.

• Competitive salary of £28,041.00 + £51.11 sleep-In allowance
• Full-time (37), permanent
• Work Pattern: The scheme is covered 24 hours, 7 days a week so you would be expected to work on a 6-week rolling rota which would involve: days, evenings, weekends, and sleep-ins (additional payment of £51.11 for each sleep-in shift)
• An enhanced DBS is required for this role which we will pay for
