About the team
The Services for Young People (SfYP) Special Educational Needs and Disabilities (SEND) Team provides a range of specialist services for young people (aged 13-25) with additional needs/SEND and those who are Leaving Care. This involves providing careers, information, advice, and guidance to support them in making positive decisions about their futures; maintenance of education, health, and care plans for those aged 16+ in further education and support to find paid employment or other positive activities.

The ‘My Future’ Programme is being delivered in all four of the Hertfordshire Colleges and has been designed to support young people with Education, Health, and Care Plans (EHCPs) to transition out of college into a positive destination or opportunity.

We are currently looking to recruit a full time ‘My Future’ Coach, to work with the colleges in the South & West of the county. The key role of the ‘My Future’ Coach is to provide transition coaching assistance to young people with special educational needs and disabilities (SEND) who are moving on from college, working closely with parents/carers.

About the role
This role will require you to:

Engage effectively with a caseload of young people with EHCPs, identified by the allocated colleges and SfYP CEIAG Advisers to provide transition coaching and support.
Work with college staff and SfYP staff to ensure that young people on caseload are supported appropriately to move into positive activities of interest when they leave college.
Support young people to take up positive activities, volunteering, or paid employment opportunities by using tried and tested supported employment techniques.
Provide advice, support and disability awareness training as required to opportunities providers to unsure they understand how to support the young person they are providing an opportunity to.
Provide information and support to parents/carers to raise aspirations and expectations for a fulfilling life after college that can include positive activities, volunteering, community inclusion and paid employment.
Work with local third sector organisations to identify opportunities that match the skills of young people on caseload.
Work to agreed targets including college leaver NEET prevention, monitoring of caseload progress through regular review, recording of data on management information systems to appropriate timescales and standards, producing reports as required.
About you
Essential:

Experience of working with young people with additional needs/SEND.

If you want a job with variety, then this role is for you, as no two days are the same. The job combines providing transition support for young people with marketing them to employers, community providers and third sector organisations.

The postholder will have the opportunity to make a positive impact on the lives of young people with SEND and their families by helping them to find suitable post college opportunities which may include community activities, volunteering, or paid employment where appropriate.
