To work within NMC Code of Conduct and to agreed Practice standards and protocols Delivery effective implementation of management of patients with Chronic Diseases as detailed by the Quality and Outcomes Framework, local quality schemes and national quality schemes Support the implementation of evidence-based guidelines in conjunction with the wider team. Ensure that all nurse-related Care Quality Commission evidence table indicators are compliant, demonstrating evidence, review, and audit cycle Participate and engage in the practice appraisal process working towards a personal development plan, utilising a reflective approach to practice. Actively participate in multi-disciplinary clinical and practice meetings. Support and participate in the development of clinical and practice protocols. A range of clinical nursing duties in line with the practice nursing team. Provide Chronic disease management for Diabetes, COPD, Asthma, and CHD/CVD Provide adult and child immunisations Ensure safe storage, rotation, and disposal of vaccines and drugs Undertake venepuncture and electrocardiography Support patients to engage in a healthier lifestyle with evidence based advice and signposting to appropriate support. Deliver travel health care, including vaccinations and advice for patients Provide high quality evidence-based care. Undertake a variety oof research and analysis tasks associated with the improvement of clinical care, medical diagnosis and treatment. Maintain and update disease registers. Promote and support a learning culture within the Practice and assist in clinical instruction, mentoring and supervision of trainees. Establish and maintain highly effective communication pathways with all the practice team, patients and visiting team members such as district nurses, health visitors etc.
