Summary

A Vacancy at Norfolk Community Health and Care NHS Trust.


To ensure the effective strategic development, management, and delivery of services within the portfolio in line with organisational and Place objectives, working in close cooperation with all strategic partners, commissioners and organisational leads.

To work with Place Directors and Clinical Directors to ensure effective delivery of the long-term conditions and specialist nursing services across the whole Trust, including providing line management to leads across multiple services.

To provide strategic clinical leadership with a pathway approach within the long-term conditions teams across the Places.

To lead service / pathway developments across the organisation and with partner organisations.

The post holder will work closely with other Heads of Service, Service Managers and Team Leaders and be proactive in ensuring environments where staff can be actively involved in the development, delivery, management and leadership of services.
• To work with Place strategic leads to ensure the strategic planning, co-ordination and monitoring of the development and delivery of a portfolio of services, ensuring clinical quality and standards are met at all times.
• To work in partnership with service leads, partner organisations and service users to develop and ensure effective development and delivery of services that will improve outcomes for all services as identified within the service specifications.
• To work with Place strategic leads to ensure the strategic planning of services that are within the portfolio, ensure that they work towards their potential in improving best outcomes for service users.
• To be the budget holder, responsible for all allocated budgets within the portfolio of services ensuring that finances are rigorously monitored to ensure appropriate use and balance of funds.
• Ensure that processes are in place across services that ensure implementation of clear safeguarding and governance procedures.

• To be responsible for ensuring effective and robust processes are implemented to manage information and data across services that meet local and national requirements.
• Ensure effective processes are in place for service evaluation that identify and produce evidence of effectiveness.
• Develop the provision of services within the portfolio ensuring all opportunities are exploited to grow the services.
• Work on service modernisation, ensuring that professionals working with the service user have the right level of expertise to deliver a safe and effective service.
• Identify clinical and service development opportunities, creating an environment that fosters positive change.
