Our dedicated Active Health team are looking for an Active Health Area Hub Coordinator to join them to work in the West of Wiltshire.

The Active Health programme is a very successful project offering physical activity to customers that have been referred to manage a medical condition.

As an Active Health Area Hub Coordinator, you will be responsible for overseeing the successful delivery of the Active Health project, communicating with a team of Physical Activity Referral Instructors based in the West of Wiltshire. You will be responsible for ensuring that all records, data and administrative tasks are kept up to date and accurate for reporting purposes.

Our ideal candidate will hold a Level 3 GP Referral qualification and preferably, Level 4 qualifications in BACPR Cardiac Rehabilitation, Postural Stability and Exercise After Stroke. Knowledge and understanding of the health referral process, experience of working with communities and excellent interpersonal skills will be essential.
