Summary
Manage referral of patients who are identified as having a clinical requirement and identifying when a patient requires medical attention. Understanding the management of patients undergoing a worsening of their condition utilising a recognised pathway for referral if appropriate. Develop clinical skills in history taking, clinical examination, assessment and/or treatment of respiratory disease Involve patients, carers and relatives in the giving of information regarding the project. Impart specialist knowledge, advice and support to colleagues, patients, carers and relative where appropriate. Use the expertise and knowledge of professional colleagues, acknowledging the boundaries of different roles. Undertake training to be able to complete patient assessment, shared decision tool and be able to facilitate appropriate on-going referrals. Ability to have effective and compassionate communication, sometimes under complex circumstances. Provide sensitive, complex information to patients, carers, relatives and professionals regarding the project. Proactively manage a referral, tracking and outcome process for CT scans and other tests Ensure onward referral to other relevant care providers ie smoking cessation, pulmonary rehabilitation, etc Understand the processes involved in evidence based care and have knowledge of where to obtain evidence. Work collaboratively with the professional agencies to ensure patient needs are met. Establish and maintain effective communication with patient, cares/relatives and multi-disciplinary teams Observe the provisions of and adhere to all Trust policies and procedures. Actively participate in the annual performance review to identify personal development needs. All employees are individually responsible for the prevention and control of infection within their own area. The employee must attend Infection Control Induction training and mandatory Infection control training. The employee must follow all Trust policies, procedures and guidelines relating to Infection Control. Adhere to the relevant Code of Practice of NMC. Exercise judgement, discretion and decision making in clinical care in line with the project protocol.
