Summary
We are a multi-disciplinary team consisting of Community Paediatricians, Nurses, Specialist Practitioners, Psychologists, Family Support Workers and Healthcare Assistants. We work together to ensure that every contact counts for the children under our care.
We provide assessment and support for children with the following specialist needs or conditions:
• Developmental Assessment - We see preschool and school age children for developmental assessment when there are significant concerns regarding their development or possibility of severe disability.
• Autism Spectrum Disorder (ASD) - We provide multi-disciplinary specialist assessment along with Speech and Language Therapists (SALT), Psychologists and Occupational Therapists (OT) for preschool children.
• Physical disability - We provide initial assessment and ongoing support along with Physiotherapists (PT), Occupational Therapists (OT) and Speech and Language Therapists (SALT).
• Child in Care (CiC) - We conduct Initial Health Assessments and Review Health Assessments, working jointly with our specialist nursing team.
• Sensory Impairment - We provide multi-disciplinary and multi-agency clinics for children with hearing and visual impairment, jointly with specialist teachers, Speech and Language Therapists (SALT) and Orthoptists.
• Down Syndrome clinic- We provide developmental support for children with Down Syndrome (DS).
We are delighted to offer an exciting opportunity for a Nursing Associate to join our Community Paediatric Team based within Peterborough.
The post holder will provide quality evidence-based nursing advice, support and care for service users with or without learning disabilities & physical health needs as well as their carers within a multidisciplinary setting.
They will be responsible for the assessment, planning, implementation and evaluation of nursing care in line with the agreed NMC standards specific to the role, and will have a working knowledge of local, national policies and legislation which govern current service provision.
The post holder will be responsible for identifying and contributing to the training needs of junior unregistered staff, and ensure that issues of equality, diversity and rights are dealt with in accordance with good practice and legislation.
They will ensure that effective communication pathways exist, and that they act within their sphere of responsibility. Supporting and delivering high quality, skilled care to identified children and young people in the clinic setting, their own homes, nursery or school settings, but will also experience working in alternative settings to ensure that the physical health needs of children and young people are met.

1. Work as an active member of the Children’s Community Paediatric Team to support and promote continuity of care established for children, young people and families on the caseload.
2. Develop an understanding of the health promotion priorities. Identifying and utilising opportunities to promote these within the workplace and during client contacts.
3. Deliver high quality, compassionate care under the direction of a Registered Nurse (or other registered care professional dependent on setting) with a focus on promoting health, independence, comfort, dignity, privacy and quality of life for patients.
4. Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual.
5. To act as a chaperone for vulnerable children and young people as per chaperone policy.
6. To promote carer and service user involvement within the service and for its provision.
7. Work with a mentor to take responsibility for continuing development of own clinical competence, leadership and reflective practice skills within the workplace.
8. Provide skilled nursing care and clinical interventions to children and young people within their own home, or other applicable venues such as outpatient clinics, to ensure families are supported in the care of their children at home under the direction of a Registered Learning disability or Children’s Nurse.
9. Complete accurate client records in line with CPFT policy. Be aware of the requirements of both paper and electronic record keeping on SystmOne and other external health care systems.
10. Maintain excellent communication with the child and family on the caseload and act as their advocate to ensure that they have ease of contact with the team, appropriate information and support. Records should reflect the child’s care, present condition and progress and be available to colleagues.
