Summary


The post holderis responsible foroverseeing the management of the ClinicalInformaticsAuditteam and the health recordsprocesses and team. Inadditiontheyare responsible for:

• Providing leadership, through their conduct and actions as per trust values acting as an ambassador for the clinical quality strategy. Provide direct line management,supervise,coachand develop staff in theCIA and records teams.

• Oversee thequality andproductionofallclinicalaudits, ensuring auditmethodologyand risk management approaches are followedby the CIAteam.

• Develop andfollowclinically crediblearchitecturefor future digital reportingrelatingto clinical auditand clinical dataand implement a monitoring process that evaluates improvements and or change to practice.

• Produce detailed and relevant reports for presentation to clinical governance group, medicine optimisation group and other forums to provide trust assurance on the quality of care

• Produce audit papers for local and national publication through HQIP

• Undertake the monthly review of the NICE, NCPOD, HQIP and other national guidancefordistillation and review by subject matter experts,prepare quarterly reports for CGG onactionsand monitoring of impacts.

• Is clinicallycredible andable toliaise with senior clinicians,facilitatingreview anddevelopment of audits and standards as well as support processes for system evaluationfollowing improvement interventions

• Provide a resource to operational and other teams to support clinical audit

• Provide a link for acute trusts to share audit information and learning

• Undertake investigations both clinical and non-clinical as necessary

• Responsible for ensuringthatIG practices are adheredtoinclinical audit, data share,and records management.

• Represent the trust at national forum re clinical audite.g.,NASQG


Core Responsibilities


Communication and Relationship Responsibilities


The post holder will ensure clear communicationsbetween the CIA and health recordsstafffostering the trusts values throughone teamleadershipand management.They will ensurethe department’s Staffareinformed ofobjectivesand key milestones.

• Work with HR and recruitment business partners to support staff and follow organisational policy and procedures. Support with investigations and fact find asrequired.

• A key relationship is with the Head of Clinical Effectiveness and Governance the post holder will provide assurance relating to the quality of clinical audits produced; clinical aspects of reports are sound and that the audit programme is delivered.

• Form partnerships with localClinical Governance leads,and Clinical Development Mangersadvisingonimprovementsfollowing audit resultsfor trust and local levels

• Work with systems and development teams to support the development of theinformationportaltomaintainthe un-finalisedePRto minimum levels.

• Write and present trust level reports for CQDF and CGG

• Provideaudit advicefor thedevelopment of theePRand PCRensuring this innovation continues to be current and relevant.

• Ensure the production of records guides to support the heath records team in undertaking their roles

• Link with IT in the development of theOnbaseand‘Readsoft’programmes

• Oversee external requests for clinical information with acute trusts building relationships that enhances information sharing and learning

• Provide local operational management teams and staff with structured reports, outlining local clinical performance, (ACQI andlocalaudits).

• Providechallengetodecisionsand escalatethrough reports ifpatient safety is not taking primacyas re-audits highlight such practice.

• The post holder will have a key role in workingwith other clinical directorate team members to support the delivery of the clinical strategy. The production of quality information that outlines the care delivered by the trust

• Represent the directorate and line manager at local and regional meetings where clinicalquality governance and learning from incidents are related.

• Liaise with communications teams to ensure audits are published and accessible to staffand key learning highlighted

• Locally manage the budget; sign-off of expenses, timesheets and authoriseovertime.

Analytical and Judgemental Responsibilities


The post holder will undertakereview of clinicalauditreports highlighting relevanceundertakingthe evaluation andquality review of all clinical audits prior to publication.

• Theyare responsible forthe review and update of health record completion guides and clinical auditand record keepingpolicies.

• They will evaluate the impact ofePRand review the access to the portal, review of allunfinalisedrecords and creating reports

• They willidentifyincidence of patient safety or otherrisks;escalate these to clinical and operational teams to ensure patientsremainsafe in our care.

• Undertake a review of all clinical audits and issues from clinical cases toprovide recommendations for the trust to adopt for improvement

• Review of SOP and system processes to ensure they are current and fit for purpose

• Provide guidance on the use of data presentation and relevance in use of graphs / infographics


Planning and Organisational Responsibilities


• Plan own workload andsupport others to deliverobjectives.

• Undertake meaningful staff PDR and review ensuring the teams are well developed and fit for the role.

• Ensure there is adequate staffcapacityto undertake theaudit programmeor other quality standards,escalating to line manager when there are risks or other challenges.

• Develop a planner for and deliveryofreports and updatesto ensure these aretimely,instate monitoring of actions for improvement as requested

• Oversee the adherence to the clinical audit programmemonitoringmonthly and adapting to organisational requirements

• Direct the processing of clinical records both current and archivedensuring the system is functional

• Organise meetings with internal and external partnersincluding:team meetings and CQDF and HQIP (YEARN)

• Ensure relevantreportsareavailable for trust meetings


Policy and Service Development Responsibilities


• Work with theHofCEto developpolicies related to audit and health records

• Plan and implement service developments for the local improvement of ACQI/medicines and other clinical audits. This will include outliningservice developments for training and education or changes to local practicewhereoutlined within findings of an investigation

• Responsible of highlighting good practice for Ambulance Systems Indicators (AmbSYS) and Clinical Outcomes (AmbCO) and sharing learning with others

• Ensure youprovide the opportunity for the teamsto developby undertakingqualityappraisals.


Financial, Physical and Information Resource Responsibilities


• To ensure all NHS resources are utilised to best effectincluding use of staff timee.g.use of Ms Teams for meetingslimiting travel

• Signoff of travel and other expenses; manage overtime, keeping within budget

• Ensure requests for data and reportsareutilisedeffectivelyand are shared with relevant teamsthrough access direct via CIA folders etc.

• Follow Trust procedure in relation to the reporting of adverse incidents of reported equipment failure, drugerror, and other adverse events.

• Prioritise ownwork loadwith that of the teamto provide efficient cover for the role and achieve Trustobjectivesfor audit and records processing

• Develop a structure to work delivery that maximises efficiency and supports the staff to undertake their role in the best possible environment


Research and Audit Responsibilities


• Support the development of systems to capture and audit information that highlights clinical performance issues

• Provide clear direction for thedevelopment ofthe Clinical audit programme and the support of clinical audit

• Undertake audits of SOP adherence, records use and department processes

• Toremaincognisant of all updates and research and developments being undertaken by the trust related to clinical practice

• Support the YAS research department in providing information and gathering data



Leadership, Management, Training and Supervision Responsibilities


• MaintainAuditexpertiseand professional credibility within the role, anddemonstratean awareness of current practice

• Demonstrate ability andexpertisein undertaking clinical investigations through clinically relevant and current records review, clear questions, supported staff, well-informed reports

• To develop and professionally present information ata number offorums including coroners; family briefings and mortality and LIR groups.

• To provide a local leader presencesupporting IPC and inspections for improvementand other service audits

• Support and develop the teams of staff theyare responsible for, ensuring they are supported in their role. The working environment is safe and adheres to working practices
