Summary
We have 5 exciting opportunities available within the Transfer of Care Hub for therapists with expertise in community care and rehabilitation to support Acute to community discharges.
The ToCH Therapy Site Lead will provide senior therapy and clinical leadership and expert advice to staff pertaining to all matters of discharge/transfer to ensure high quality and safe patient flow with specific responsibility for identifying and facilitating the “pull” of patients in pathways 1 and 2 directly into SCFT community services.
The ToCH Therapy Site lead is a key member of the TOCHs and will work closely with the Site Capacity and Flow Lead, Capacity and Flow Head of Service and the TOCH leads to develop a strategy to improve patient experience and outcomes of discharge /transfer by focusing on system wide data and patient and staff feedback.

Worthing General Hospital (Worthing)
St. Rischard’s Hospital (Chichester)
Sussex and Surrey Healthcare Trust (Redhill)
Royal Sussex County Hospital (Brighton)
You will play an active role in the development of the service through reflective practice, clinical supervision, and quality improvement initiatives. This is a fantastic environment in which to develop your leadership skills through formal and supported training, using these to assist in shaping future clinical practice of the team.
a) To lead, oversee and drive forward effective and efficient patient flow, ensuring continuous improvement across pathways 1-3 at a named hospital site.
b) To provide subject matter expertise in Intermediate Care pathways on discharge from hospital. To include home first, rehabilitation, reablement and recovery services.
c) Provide clinical leadership to embed a consistent ‘get you home approach’ with clinicians and operational leaders as the default discharge pathway from hospital.
d) Drive forward a ‘home first’ culture across acute and community teams, encouraging collaborative working and trusted assessment processes between therapy teams to increase productivity and improve patient experience and continuity.
e) Attend/lead the named site operational and performance meetings including daily calls /huddles (for all pathways).
f) To work collaboratively with external stakeholders and partner organisations to address urgent care local pressure, system surges and escalation ensuring capacity is fully utilised.
g) Provide leadership, support and education to UHSx discharge co-ordinators and SCFT in reach practitioners.
i) To support ward staff, patients and their families to agree discharge/transfer plans providing practical solutions to prevent unnecessary delays.

The role involves managing and coordinating a wide range of complex and competing priorities regarding discharge/transfer and necessitates working closely with a range of health and social care system partners.

Delegate or follow up on specific actions following these meetings.
f) To work collaboratively with external stakeholders and partner organisations to address urgent care local pressure, system surges and escalation ensuring capacity is fully utilised.
g) Provide leadership, support and education to UHSx discharge co-ordinators and SCFT in reach practitioners. Enhancing the intermediate care and community service knowledge base throughout the team and ToCH. Including the General Virtual Ward offer.
h) Offer regular updates or education sessions to acute teams to enhance knowledge and understanding of community services.
i) To support ward staff, patients and their families to agree discharge/transfer plans providing practical solutions to prevent unnecessary delays.
j) Work collaboratively with acute clinicians and community discharge assessors to provide expert case management support or review for complex cases and patients with a high level of need.
k) Support the consistent completion and use of clinical tools to describe need at ward level. To then support the clinical discussion and stratification of discharge pathways consistently within the ToCH.
l) To contribute to the development of the Trust’s operational policies relating to patient flow and discharge.
m) Collect and analyse discharge, clinical tool and patient need data. Contributing to wider capacity demand workstreams and subsequently describing the future of community service and workforce options or innovation.
