Summary
To track patients pathways on a daily basis (focus on day 55 onwards and diagnosed patients particularly) Escalate issues or delays within the patients pathways after day 5 Ensure that Somerset (Cancer Tracking System) remains up to date To act as co-ordinator for the Multidisciplinary Team Meetings in the absence of the site specific MDT Co-ordinator To facilitate collection of data for all cancers and enter this onto an appropriate database To prepare and maintain all required and related documentation pertaining to the efficient and effective functioning of the MDT Meeting To ensure that individual patient treatment/management plans are recorded in the patients notes and that these plans are disseminated to the patient and their General Practitioner within the stipulated time frame To be responsible for the making of appointments, notifying patients and liaising with practices as required by the Cancer Data and Information Manager To ensure that MDT administration meets Trust Operational Policy To highlight capacity issues to Cancer Data and information officer in a timely manner.
