Summary
The main duties of this role will include: a Daily contact with patients and their carers; leg ulcer management nurse team and wider Tissue Viability team; community nurse teams; primary health care teams; colleagues within the Trust Provide evidence-based information to patients and their carers and multi-disciplinary colleagues specific to the short-term management of complex leg ulcers. Advise on treatment plans in line with national guidance. Make recommendation on discharge as to after-care hosiery and promotion of skin integrity. Empower patients to better understand their condition and methods to promote healing including dietary needs, leg elevation, exercise and mobility Sign-post and refer patients/carers appropriately to support their needs and ensure positive outcomes for skin integrity Work collaboratively with patients to enhance concordance for those who require support to better manage their condition and engage with treatment plans. Discuss with patients their treatment plans including the need to regularly attend on time at the clinic. Be aware of impact on the efficient running of the clinic and reduce instances of clinic running late. Report clinical and non-clinical incidents and proactively manage risk to vulnerable patients The team provide regular leg ulcer training to healthcare professionals and carers for which we provide support and training to co-facilitate with other nurses within the Tissue Viability Service. Create an environment that is conducive to learning and development using evidence-based practice to develop the skills and knowledge of students and others who attend the clinic to develop skills in the management of venous leg ulceration
