Summary
Provide a comprehensive specialist assessment of patients who are referred to the service, often with complex presentations, in a holistic manner using investigative and analytical skills. Apply advanced clinical reasoning to devise effective rehabilitation treatment plans seeking guidance from the senior physiotherapist where applicable. Implement effective, evidence-based treatment using a variety of treatment approaches, and evaluation of treatment using standardised outcome measures. Record all activity and patient documentation on the RiO system using a mobile laptop device. Be professionally and legally responsible and accountable for all aspects of your professional activities working within codes of practice and professional guidelines. Demonstrate physical ability to carry out physiotherapy assessment and interventions, including highly developed manual physiotherapy techniques and therapeutic handling. Respond to unpredictable work patterns where there is a requirement for the post holder to concentrate at high levels for frequent periods throughout the shift both during patient contact and when dealing with other elements of the role. Communicate effectively with all patients, carers and other agencies involved in patient care. Be responsible as an autonomous practitioner to undertake a significant clinical caseload. Provide clinical supervision and facilitate reflective practice to junior team members and student physiotherapists. Liaise effectively with all members of the multi-disciplinary team and work effectively as part of this team, acknowledging each disciplines roles and responsibilities. Support senior members of the team with audit and service development activities where appropriate. Keep up to date with clinical developments and maintain background knowledge in the speciality of stroke, analysing current research and discussing and implementing changes in clinical practice. Attend and contribute to in-service training programme and team meetings. Be responsible for the safe use of equipment used in carrying out physiotherapy duties. Take an active role in screening all referrals, accepting all those which are appropriate and transferring/returning any that do not meet the criteria. Work towards achieving all local elderly, frailty, stroke and neuro specific competencies required within the role. Support the integration of local NHS services, local authority and third sector. Participate in the development of appropriate guidelines, protocols, audit & evaluation. Identify any gaps in the service and report to the Team Leader/ Service Clinical Manager. Work proactively in seeking the views of others about the service provided. Identify need, plan, devise, deliver and evaluate training programmes with a specialised content. Share specialist knowledge with colleagues at a local level i.e. Clinical Excellence Networks, Multidisciplinary Team Meetings Act as student supervisor/assessor under the guidance of the student co-ordinator and provide observation sessions. Offer specialist clinical support and acts as a mentor to the less experienced clinicians. Participate in non-managerial clinical supervision.
