Summary
Job Title: Visiting Advanced Clinical Practitioner (ACP) 36 hours p/w Reports to: PCN Clinical Director / Lead GP Location: Across member practices within the Wyre Forest PCN Salary: TBC Hours: 36 Job Summary: The Visiting Advanced Clinical Practitioner (ACP) will work across the Wyre Forest PCN to deliver high-quality, patient-centred care in patients homes or care settings. This role focuses on providing clinical assessments, advanced decision-making, and management for housebound, frail, or complex patients, helping to reduce hospital admissions and support proactive, preventative care in the community. The ACP will operate with a high level of autonomy and clinical responsibility, working collaboratively with the multidisciplinary team across PCN practices. Key Responsibilities: Clinical Practice: Conduct comprehensive clinical assessments, including physical examination, diagnosis, treatment, and prescribing. Undertake home visits for acutely unwell, frail, or complex patients. Monitor and manage long-term conditions, polypharmacy, and treatment compliance. Lead on the development and implementation of personalised care and support plans. Work to prevent unnecessary hospital admissions and facilitate early discharges. Leadership & Collaboration: Collaborate with GPs, nurses, pharmacists, care coordinators, and community teams to deliver integrated care. Education & Development: Maintain up-to-date knowledge and participate in continuing professional development. Support service improvement and clinical audit across the PCN. Professional Standards: Maintain accurate, timely, and comprehensive clinical records. Adhere to relevant clinical governance frameworks, safeguarding protocols, and prescribing policies. Uphold professional codes of conduct and registration requirements. Person Specification: Essential: Registered Nurse MSc in Advanced Clinical Practice - essential Independent Prescriber (V300) Proven experience in primary or community care settings - essential Strong assessment and diagnostic skills Full UK driving licence and access to a vehicle Desirable: Knowledge of local health and social care pathways Familiarity with EMIS Why Join Us?
