Summary


We have an exciting new opportunity for a Band 7 Occupational Therapy within the new Hounslow Post Covid Community rehabilitation Team.

Our Vacancy is for Fixed Term Occupational Therapy– Band 7 – 37.5hrs

This is an ideal post for an experienced Band 7 Occupational Therapy looking to further develop their specific therapeutic and management skills or a current Band 6 looking for a new challenge.

This job will allow the successful applicant to work within a specialist Post Covid service with patients with a variety of clinical conditions. They will be able to take ownership of a non-complex patients with Long Covid symptoms of their own whilst working in collaboration with a variety of clinical specialists within the team.

Within the team there is a strong focus on personal and service development, and you will be given opportunities to participate in in-service training programme and external training where appropriate.

This post is currently fixed term until the end of March 2026, however there may be scope for further extension following a successful pilot of this additional support.

Independently managed a caseload of complex cases and work alongside other Post Covid clinical leads, sharing overall responsibility of the caseload management of the service.

Work in the Integrated (joint) Triage Service with acute staff to screen and assess referrals into the service, including gathering medical history from GP records/referrers and identifying if appropriate for Post Covid services or not; establish needs priorities and assign to appropriate pathways.

Undertake advanced assessment of people with Post-COVID Syndrome including complex multi-pathology combining both physical and mental health illness using a range of assessment tools such as comprehensive medical history, Physical assessment screening, Appropriate Patient Reported Outcome Measures (PROMS) for e.g., fatigue, breathlessness, mood, cognition and functional ability.

Formulate and evaluate holistic management and person-centred treatment plans, using clinical reasoning and utilising a wide range of treatment skills and options to formulate a specialised programme of care e.g., supported self-management.

Ensure that the service provided is patient centred.
