Summary
The post holder will act as a clinical leader across the locality. The Post Holder will work with The Operational lead, Team Manager and transformation leads to develop and support seamless pathways, both within and relating to the CMHT, to create a smooth transition for the service user based on clinical need. Implement quality improvement initiatives within the team with a focus on patient safety. Provide clinical advice, guidance and support to the community team through supervision, appraisal and attendance and facilitation of the MDT meeting. To provide cross cover for the Clinical/LCP lead and Community Mental Health Team Manager To lead in ensuring the teams understanding and adherence with the Trust KPIs and seeking creative and collaborative ways to enhance this when there are challenges in compliance To ensure the CMHT has the opportunity and encouragement to engage in regular CPD and reflective practice To support in the assessment, development and implementation of specialist health and education programmes Alert managers/ Operational leads to resource issues which affect learning, development and performance Identifies own and others personal developmental needs against the outline of KSF, to be able to evaluate own knowledge and practice Encourages others to make realistic self-assessments of their application of knowledge and skills challenging complacency and actions which are not in the interest of the public and/or users of services Actively promotes the workplace as a learning environment encouraging everyone to learn from each other and from external good practice To actively attend and contribute in forums/ workstreams and meetings that promote service review and improvement, encouraging team involvement and reciprocal feedback mechanisms Managing and developing care pathways To ensure the delivery of targets/ objectives/ outcomes within the service by putting in place systems and processes to monitor and manage performance in accordance with the Trusts Performance Management Policy To identify effective and efficient ways of working which improve health care and motivate a team or individuals to implement this Responsible for ensuring the MDT negotiate and oversee the development of individual and highly complex care programmes in accordance with service users, using a high degree of diplomacy and motivational skills Ensure the promotion of Social Inclusion and co-production with a range of stakeholders including service users and the voluntary care sector To identify and interpret the needs of patients and service users into health improvements Encourage team members to find new ways of developing and delivering services Respond to clinical emergencies and ensure that staff groups are aware of their obligation to do so Develop quality team indicators to support performance management reporting to demonstrate quality and outcomes shown through audit and evaluation of evidence-based practice Ability to produce reports and audits and demonstrate quality using both qualitative and quantitative approaches to senior managers Audit the standards of record keeping and documentation, providing learning feedback to individuals and the team, to maintain high level of clinical standards Identify and address own professional supervision needs Provide clinical leadership and role modelling for junior staff in relation to risk assessment, management and risk taking and therapeutic clinical interventions Undertake first line clinical leadership responsibilities which will include appraisals, the development of personal development plans and professional development of staff in the team Participate in the recruitment and selection of staff and ensure completion of the induction programme agreed for the service Undertake and support clinicians to complete reports for complaints, Serious Untoward Incidents (SUIs), projects etc as required Provide clinical leadership in mental health assessment and review processes and home treatment To take responsibility for the teaching, assessing and support of students within their community placements, meeting the aims and objectives of the university / college, where needed.
