Summary
Clinical responsibilities: provide medical advice and support to the St Margaret's CNS team in the community settings with a focus on West Somerset including Minehead and Taunton encourage a holistic response to the care of patients and their families Deliver supervision and education to medical students and postgraduate doctors in training, nursing and allied health professionals support Somerset End of Life individualised care planning This is a 7 PA post that can be worked flexibly (including flexible working days) to complement current work. The post holder will work alongside the MDT to support the St Margaret's Hospice community team. These sessions are used for support of a dedicated community based clinical nurse specialist team via case reviews. It also includes the ability to review people within a specific geographical area in their own homes. A typical session may include 10-20 CNS led case reviews and 1-3 domiciliary visits depending on needs on that day.
