Summary

Hep C Outreach Support - Fixed Term 12 Months

Working hours: 37 hours Monday-Friday
Location: You will based at both 30 Manningham Lane, BD1 3DN and Temple Street, Keighley, BD21 2AD

About the role...
An opportunity has arisen to support the Hepatitis C elimination agenda as part of the Waythrough Hep C team in a post hosted by the West Yorkshire Liver Care Network. This will initially be funded for 12 months with the opportunity to extend for another 12 months subject to funding becoming available.

Some key duties will include:
As directed by Hep C Regional co Ordinator and working within the Waythrough Hepatitis C team based at New Vision Bradford, the role will ensure that Hepatitis C and other BBV interventions are being delivered consistently.
There will be a focus on those who are complex, disadvantaged and struggling to access testing, treatment and interventions to raise awareness of transmission and reinfection.
There will be a requirement to outreach those not engaged with drug treatment services at Bevan Healthcare and including ODN home visits and liver surveillance events across ODN.
Working with ODN nurses and Hep C Trust staff and peers will ensure that marginalised individuals not engaged with other services are afforded equitable care.
About you...
You must have a valid driving licence and access to a vehicle or the ability to travel via public transport efficiently, to be considered for the role.
Lived experience would be an advantage but not essential.
You will need to have excellent communication skills.
You will have sound knowledge of Hep C and the elimination criteria and a passion for working as a team to deliver exceptional service as part of the Hep C elimination programme.
