Summary
Derbyshire Community Health Services currently has an exciting opportunity for a Support Worker Supervisor to join the Community Response Team on a full time bases.
The Community Response Team support discharges from the acute hospitals, providing short-term and reablement care.
Previous caring experience is essential.
Your base would be Walton Hospital, Chesterfield.
You will need to be flexible and committed to caring and supporting people at home with care needs.
Ability to travel to any site where DCHS services are provided
Shifts follow a 7-day rota including weekends and bank holidays.
You will be supported and receive training to help you develop.

To be responsible for the day-to-day supervision of support workers.
The delegation of patient’s visits to ensure an effective support worker service is provided to patients in a timely and cost-effective way.
To be a role model to the support worker team and actively monitor and maintain clinical skills and professional manner.
To Communicate effectively with patients and carers to maximise rehabilitation potential, as appropriate and under supervision.
To work with qualified therapists and nurses in the assessment and treatment of patients with a variety of conditions and progress/modify intervention programmes within parameters set by the therapists and nurses.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
