Summary
To provide care in both the acute and community setting for people with diabetes including clinics in GP surgeries, health centres and patient homes To work within a multidisciplinary team alongside consultants, nurses, podiatrists, dietitians and educators To deliver both patient and health care professional education sessions in accordance with the service level agreement.
