We are looking for someone who is proactive, approachable and flexible to join our established team working with patients experiencing Long COVID symptoms. This is a short-term post and offers a great opportunity to learn about Social Prescribing whilst supporting individuals impacted by long-term health conditions.
Long COVID affects people physically, mentally and emotionally. People experience it differently, and as a condition, it is still relatively new to patients and professionals. In this role, you will signpost and refer patients to services so they can better self-manage their condition and its impact.
You’ll coordinate activities for patients through six weekly sessions, progressing them through their journey and initiating interventions. You’ll ensure that patient notes, results and reports are made available to the appropriate clinician in time for the next step of the pathway.
You will also arrange and facilitate online group sessions to enable peer support.
Applicants must have relevant personal or professional experience of working with vulnerable adults or patients in social care, health, education or voluntary and community sector. You’ll need an understanding of the impact of Long COVID and be able to motivate and empower others. You should have excellent written and verbal communication skills, and be confident facilitating group sessions.
This is a 1 year fixed-term position. There is flexibility for where this role is based in Kent. Immediate start available and a car driver is essential.
