Summary


Are you looking to develop your Physiotherapy skills in the community?

This role offers an excellent opportunity to work as part of a multi-disciplinary team delivering client centered evidence-based care.

This is an exciting opportunity to join a supportive and innovative multi-disciplinary team.

You will have a role developing your Physiotherapy practice and deliver evidence-based rehabilitation and assessments for clients within the community.

You will be given the opportunity to build and maintain key relationships with acute and community referrers to the services as well as partners in the community.

This post holder will be responsible for the supervision of junior staff, rehabilitation assistants and students, participate in any audit or development being undertaken by the service.

To assess the functional needs of patients providing intervention plans based on protocols and pathways aimed at improving function and reducing the impact of disability, working with clients to identify PT goals as part of their overall care plan.

To independently manage a delegated caseload of straightforward and complex cases demonstrating a high level of problem solving, clinical reasoning skills and professional judgement.

To manage patients across their care pathway from hospital to home by referring to and liaising with appropriate services, taking on the role of case management for some patients

It is essential that you have current HPC registration and recognized qualifications as a Physiotherapist.

You should have experience working with a variety of patients and rehabilitation experience.

Can clearly demonstrate skills for specialist assessment and treatment of clients with a range of presentations and comorbidities, and a variety of other conditions (including MSK, geriatrics, falls, Parkinson’s, amputees and orthopaedics).

Experience will include working in multi-professional teams and across a range of different agencies to achieve the optimum outcome for service users (including working in the community).

You will have excellent interpersonal and communication skills, a positive and innovative approach to service development and have a commitment to multi-disciplinary team working/skill sharing.

This is a community based role so having UK full driving licence and access to car for work are essential.
