Summary


The 0 to 19 enhanced pathway is an integral and distinct caseload within the Croydon 0 to 19 Public Health Nursing (PHN) service. A programme of care will be delivered to families and young people with additional health needs and vulnerabilities determined by inclusion criteria.

The aim is to improve health and development outcomes of babies, children and young people and their families. The tools that will be used by the Enhanced Pathway Nurses for example the Family Partnership Model and Outcome Star are underpinned by research to get the best outcomes for children, young people and families.

The Enhanced Pathway Specialist Nurses will have a proven track record as excellent communicators and be able to work in partnership with families to set achievable goals and elicit adaptive behaviour change.

The main role for the Specialist Enhanced Pathway Nurse is to be the delegated nurse for children requiring targeted and specialist support as well as contributing to the public health agenda. This is achieved through conducting thorough, individualised health needs assessments, planning of programmes of care tailored to families’ needs and effective liaison with colleagues within Croydon Health Services NHS Trust and with partner agencies.

• To deliver a programme of care to a caseload of vulnerable families, children and young people
• Carrying out ongoing health needs assessments for all children and families within the community, using professional judgement to allocate each family to the correct level of health visiting care; Universal, Targeted and Specialist pathways
• Delivering the key touch-point contacts, within required timescales, according to the service model, to meet KPI target, The post holders will be trained in the family partnership model to identify health needsand develop care plans with the families and deliver evidence-based interventions.
• The post holder will line manage a small team consisting of a Staff Nurse and a Community Nursery Nurse.
• The post holder will be supported in the development of their leadership skills through the provision of training and mentoring.
