Responsibilities
Assist qualified social workers in evaluating clients' needs and developing individual care plans.
Provide emotional support and practical assistance to clients and their families.
Conduct outreach and engagement activities to connect clients with community services.
Document interactions and maintain accurate and confidential records in compliance with regulations.
Participate in multidisciplinary team meetings to discuss client progress and strategies for improvement.
Research and stay informed about available community resources and programs for clients.
Support discharge planning and transitional care efforts for patients leaving the hospital.
Requirements
Bachelor's degree in social work, psychology, or a related field.
Strong interpersonal and communication skills, both verbal and written.
Ability to work collaboratively in a team environment while also being self-motivated.
Familiarity with community resources and healthcare systems is a plus.
Compassionate and empathetic attitude towards clients and their families.
Strong organizational skills and attention to detail in documentation and reporting.
Commitment to ongoing professional development and a desire to pursue qualifying social work credentials.
Hours Per Week: 22.50
Start Time: 09:00
End Time: 17:00
