You will accept referrals, carry a small caseload of customers, and provide intensive interventions, harm reduction, information, advice, and guidance to engage and support people with complex support and housing needs, who are former street homeless; to enable and empower them to sustain a tenancy and engage with various internal and external support services across Middlesbrough.

You will assist and empower customers to access various statutory and voluntary services to improve their circumstances and sustain their tenancy, through home/community visiting and developing a robust risk and support plan to enable customers to achieve independence.

This demanding and rewarding role requires a driven person with a sound knowledge of social and medical referral pathways and a skill for problem solving and the ability to communicate clearly and effectively, as well as respond calmly and effectively to crisis.

The right candidate will be confident, approachable and knowledgeable, calm and unflappable, and able to take control of a situation and empower customers towards independence. Ideally, you will have a sound understanding of a trauma-informed approach, safeguarding and mental capacity.

You will be able to communicate clearly and confidently with customers in crisis, as well as with fellow housing and support professionals in both the statutory sector and the third/voluntary sector.

As you will proactively support customers with complex support needs, you will be required to provide regular out of hours planned and responsive support.

The Supported Living team sits within the wider Independent Living team, who provide numerous support services to enable our customers to live independently.

Internal candidates must discuss a secondment with their line manager before applying for this role.

This is a full-time funded post until 31 March 2028.
