An exciting opportunity has arisen for a Band 3 Community Support Worker to join our Barnet Unplanned Care Community Service by providing a high quality service to our housebound patients by facilitating earlier hospital discharges and preventing unnecessary hospital admissions.

As a Band 3 Community Support Worker you would be responsible for providing safe, personalised and effective care to patients who require community therapy care by working alongside the community nursing/therapy team to deliver clinical and administrative duties.

Your main duties will include providing a number of clinical activities and holistic care under the direct delegation of a registered therapist using a wide range of clinical skills and interventions within a home environment.

You will need to possess excellent Communication skills and be a team player. You will also need to have good IT skills to support the systems in place. Having UK full driving license and access to car for work is essential.

• To contribute to the on-going assessment of a patient by completing the appropriate paperwork for allocated patients, recording and reporting all clinical and behavioural information to the nurse/therapist for action and planning purposes.
• To undertake, and report back on delegated clinical activities that have previously been signed competent. E.g.: vital signs, gait/mobility training, falls prevention groups, increasing independence through activities of daily living
• With the appropriate training the CSW will plan, design and deliver higher level strength and balance groups for patients/clients with a history or high risk of falls who have been identified as suitable for group exercise by the Falls Specialist Therapists.
• Within the remit of the higher level strength and balance group, review clients’/patients’ exercise programmes.
• Provide personal care support to frail patients.
