Summary
Demonstrate clinical excellence and maintain consistently high standards of clinical practice in palliative care. Act as a role model providing clinical leadership for colleagues. Advise and support on patients with complex needs and their families at home, maintainingan approach that incorporates physical, psychological, social and spiritual aspects of care, inaccordance with the organisation mission and values. Manage a caseload/ virtual ward bed based care of complex patients promoting the highest possible standards of individualised care, symptom management and bereavement support through a process ofassessment, planning, coordination and evaluation. Effectively allocate and support team members to implement the care plan, monitoring andevaluating the care provided, delivering practical nursing care when appropriate.
