Summary
Person requires support to enjoy going out
Preferred days/times:
Support at the weekend
2 hours on a Saturday
1 hour on a Sunday
After 12 noon, time to be arranged with the PA
Hours have potential to increase
Tasks/duties:
Supporting with accessing the local community
Emotional support and reassurance
Meal preparation
Supporting with aspects of personal care such as helping with washing their hair or upper back in the shower, occasionally supporting with toileting needs due to Fibromyalgia flares.
Domestic/household tasks
Checking that medication has been taken
PA to have a vehicle and full, clean, Uk driving license.
The client will pay mileage
Client main diagnosis:
Hip bursitis
Fibromyalgia
IBS
Acid reflux
Sciatica
Vestibular migraine
Non epileptic seizures (blackouts)
Mental health:
Emergent PTSD
ADHD
Generalised anxiety disorder
Depression
Emotionally unstable personality disorder (EUPD)
Manual Handling
No manual handling support required
Communication:
There are no communication barriers
Pets:
The client has two cats
Client enjoys:
Gaming, going out for a coffee, getting out of the house, Disney films.
