Summary


The Hillingdon Young People’s Transitional Service is offered to young people, with health needs and/orcomplex health needs who are preparing to leave children services. The Transition Nurse Specialist forchildren’s community team role is to facilitate, support and co-ordinate the transition from Children’s to transition toAdult Health Services.The role includes development of the transition service, participation in strategic expansion and collaborativeworking across health agencies , including Hillingdon Hospital , social care and education. The role includes a largeeducational component educating both Children’s and Adult Services to facilitate the transition process.

To take the lead in coordinating the transition process for young people with health and or complexhealth needs or life limiting conditions.

To compliment the current service provision within the Children’s Community Nursing Team Serviceand to work closely with the other members of the Transition.

To assess referrals to the Transition Service and where necessary direct referrer to amore appropriate service.

To ensure that a Health Summary/Health Passport is produced, in agreement with the youngperson, with identified health actions/health plan to be completed within a set time scale.

To assess the physical, social and psychological health and well-being needs of identified youngpeople giving information, advice or counselling and support and making referrals as appropriate.

To provide specific advice, training and support to staff in health and other partner agencies whoare providing care to young people in Hillingdon

We will offer clinical supervision, regular appraisal and support in achieving your personal development plan as well as peer support from colleagues in similar roles.




You will have the ability to provide case management of an identified caseload of children and young people with complex health needs by utilising and promoting evidence-based care.

You should have excellent interpersonal skills to support you in managing complex discussions with a range of agencies and individuals, including care providers, patients and families.

In addition the ability to work in partnership with parents and multi-disciplinary agencies to provide skilled care to children and young people within the community to support safe transitioning to adult health services.

You will be highly organised and have a flexible approach to work, able to deal with difficult situations autonomously with demonstrable, excellent communication skills.
