Summary
The post holder is responsible, as part of a multidisciplinary clinical team, for delivering specialist nursing care to patients with diabetes, their carers and health professionals in secondary care.
The post holder is responsible for liaising with other HCP’s both specialist and non-specialist within the Trust and with colleagues in primary care to ensure patient safety both whilst an inpatient and on discharge.
The post holder will ensure that patients receive the necessary care and treatment to enable progress along the agreed pathways of care in line with the National Service Framework.
To support and review patients admitted to the hospital with diabetes to ensure that their care and safety is maintained whilst they are an in-patient.
The post holder is responsible to manage a caseload of patients in secondary care.
The post holder works as an autonomous practitioner delivering knowledge and expertise within the specialist area .
To have good communication skills and work within the specialty team to ensure cohesive multidisciplinary management of patients
To manage a caseload of patients with T1DM and T2DM in secondary care in both inpatients and outpatient settings, ensuring continuity of a high standard of evidence based nursing care.
To attend and co-ordinate MDT meetings if appropriate to the role ensuring all patients are represented
To promote effective communication between primary and secondary care developing systems which support this.
To work alongside Community Nurses and GPs to help develop their knowledge and skills in diabetes service provision in line with the East London Guidelines for Diabetes Management.
To support the Clinical Effectiveness Group work on diabetes guidelines, audit and the diabetes.
Where the postholder is qualified as an independent prescriber, to undertake responsibility for prescribing medication in line with supplementary and independent nurse prescribing guidelines following accredited training.
To initiate, lead and develop in the delivery of programmes of education and training according to the identified needs of clients.
To organise and carry out diabetes study days and trainings for inpatient nurses, junior doctors and student nurses and doctors.
To assist in multidisciplinary development of policies, guidelines, care recommendations and protocols for diabetes care to support the implementation of the NSF for Diabetes
To participate in the evaluation of the service in terms of clinical effectiveness, clinical excellence and value for money.
