Summary
The post requires autonomous working to provide pharmaceutical care to our patients. You will have daily appointments to offer medication reviews and medication advice as well as minor alignments and other contractual obligations including QOF and IIF schemes Completing medication reconciliations and reviews for patients SMR both in person and over the telephone Complete SMR Medicine optimisations and medicine rx Carry out benzodiazepine reviews. Prescribing medication Must be able to offer sound and professional advice in relation to clinical management issues Maintain and improve quality and patient safety Provide pharmaceutical input for clinical governance meetings
