Responsibilities
Facilitate the brokerage of social care services for clients.
Assess client needs through comprehensive evaluations and consultations.

Monitor service delivery to ensure compliance with established standards and regulations.
Advocate for clients' rights and needs in all aspects of service provision.
Maintain accurate records and documentation of service agreements and client progress.
Requirements
Proven experience in social care or related fields, with a focus on brokerage.
Strong understanding of social care frameworks and regulatory requirements.
Excellent communication and interpersonal skills, with the ability to build rapport.
Proficient in assessing needs and creating tailored care plans.
Ability to negotiate effectively with service providers and stakeholders.
Strong organizational skills and attention to detail.
