Summary


To provide health improvement expertise and support people with Type 2 Diabetes through delivery of healthy living interventions. Health Coach will be helping people gain and use the knowledge, skills and confidence to become active participants in their care so that they can reach their self-identified health and wellbeing goals.

Supporting patients with 1-1 interventions for health coaching to support self-management of type 2 diabetes with the use of coaching models and motivational interviewing



Attend monthly cooking classes with patients



Support on weekly walking groups



Working alongside primary, specialist care, physical, MH services and social care services



Use of Motivation Interviewing and other behaviour changes strategies to increase patients’ skills and confidence to manage their own condition and set achievable goals, and engaging patients in making informed decisions.



Weight management, physical activity, smoking cessation and alcohol awareness support



Establish and maintain strong and effective partnerships with teams within partner organisations who can influence and deliver the Health Improvement and Inequalities work streams e.g. Health care staff, Local Authority staff andVoluntarysector



To provide Health Education to people to improve health and encourage healthy lifestyles and promote behavioural change.
