Summary
Responsible for developing, with patients, a continuing management plan for the health of the patient and when appropriate to refer to other clinicians and agencies, The successful candidate will work within the multidisciplinary team to deliver the Requirements of the QOF, DES and IIF framework as part of the primary care network, whilst also supporting the network. The role will involve independent prescribing within the scope of professional practice.
