This is a combined post with Acute (AMU) and Respiratory Medicine at the Royal Liverpool Hospital strengthening links between the departments and enhance patient care by providing respiratory specialist supporting in AMU. This post supports the expansion of the ambulatory care service and develop new pathways for patients.

The candidate will be a key figure within the existing MDT, collaborating with senior staff across the division of Medicine. This is an exciting opportunity to contribute to the provision of exceptional clinical care for patients with complex acute medical conditions, as well as to be involved in the development of the AMU and its services. A significant component of the role is dedicated to respiratory care, which will include managing both general respiratory patients and more complex cases, particularly within the newly developed NIV unit. The role offers the chance to provide input into the bronchoscopy service, specialist respiratory clinics, and continued development of pleural, ILD, pulmonary infection, and lung cancer services. The post holder will have access to research opportunities within the Respiratory Medicine department. This role will be an integral part of the consultant team, providing leadership in acute medicine, and will contribute to high quality clinical care, teaching, and research. This post is ideal for a dynamic consultant keen to develop both in the acute and respiratory medicine fields.

Respiratory Components:
• Ward management of up to 28 respiratory patients
• RSU management of 10 acute respiratory patients including 6 dedicated NIV beds.
• Opportunity to deliver bronchoscopy, general respiratory, and pleural clinics during off ward weeks
• Participation in specialist services such as Severe Asthma, Pleural Disease, ILD, Pulmonary Infection, and Lung Cancer (EBUS @ LHCH)
• Opportunities for research involvement
• Collaborative work with the community respiratory team (CRT) led by Dr Hadcroft
• On call commitment only during ward weeks.

AMU Elements:
• Provision of consultant care to all patients on the AMU, from admission to discharge or transfer
• Participation in four morning ward rounds daily, reviewing patients admitted overnight, and ongoing management of those admitted via GP or ED
• Lead day-to-day clinical management of the AMU, working alongside senior nurses and multidisciplinary teams
• Contribute to weekend and holiday cover for AMU
• Supervision of junior medical staff and participation in educational activities for trainees
• Coordination of the discharge process, including electronic discharge summaries for patients sent home directly from AMU
• Focus on ambulatory care, providing urgent medical opinions and facilitating follow up for discharged patients
• On call duty for AMU and respiratory specialties, with an on call commitment of approximately 1 in 9

This Consultant Physician role offers a blend of clinical and leadership duties across both Acute Medicine and Respiratory Medicine. The post holder will be expected to support and develop the AMU, providing consultant cover for medical admissions, both from ED and GP referrals. There will be responsibility for the oversight of all patients admitted to the AMU, including providing senior medical input to decide on patient management and ensuring they are appropriately triaged and referred to the correct specialty.

In Respiratory Medicine, the consultant will provide leadership in the Respiratory Unit, managing complex respiratory patients and supporting the newly developed NIV unit. The role also includes responsibility for overseeing pleural procedures, managing acute respiratory patients, and contributing to bronchoscopy lists. The post holder will also collaborate with various specialist services, including the Severe Asthma service and Lung Cancer MDT. There will be significant opportunities for research involvement within these specialties.

The post holder will also have responsibilities for teaching and mentorship. You will be expected to actively engage in the educational development of junior doctors and contribute to multidisciplinary teaching sessions. Clinical supervision is integral to the role, and the post holder will be a clinical mentor for one or more trainees.

As part of the AMU, the role includes leading morning rounds and reviewing all admitted patients. The post holder will supervise junior staff and ensure comprehensive and timely discharge decisions. An essential aspect of the role will be the development of ambulatory care services within the AMU, with at least one session a week focused on urgent care and discharge follow up to prevent unnecessary admissions.

The post holder will contribute to the on-call rota for the AMU and Respiratory Department, and may be called upon to support the hospital at home service and other community respiratory initiatives. The job will involve a blend of clinical duties and leadership, with an emphasis on continuous improvement of services for acutely unwell patients.
