Responsibilities Your responsibilities will include: The Enhanced Practice Nurse will play a key role in providing compassionate clinical leadership, empowering care home residents, and enhancing patient care through collaboration and innovation. Your responsibilities will include: Leading and conducting regular ward rounds in aligned Care Homes, delivering proactive and patient-focused care. Reviewing medication and working effectively within a multidisciplinary team (MDT) to ensure patients receive the best possible care. Collaborating with patients, their families, and the MDT to conduct assessments, develop and implement tailored care plans, and evaluate outcomes. Using your clinical expertise and evidence-based practice to tackle complex clinical issues. Delivering innovative clinical care within evolving care models, staying adaptable and responsive to changing needs and technologies. Educating and supporting patients and their families, providing guidance and encouragement to manage conditions effectively. Taking part in clinical audits, research, and quality improvement initiatives, ensuring our services remain cutting-edge and efficient. Following national and local protocols while recognizing your professional limits and making appropriate referrals when needed. Delegating responsibilities to other MDT members, ensuring appropriate supervision and accountability. Overseeing vaccination and QOF programmes within Care Homes Providing proactive and preventative care by visiting patients with complex needs in their own homes, ensuring they receive tailored, high-quality support in their familiar environment. Demonstrating initiative and creativity in problem-solving and enhancing service delivery. Ensuring team performance and service standards are consistently high, promoting excellence in care. Supervising and mentoring General Practice Assistants, fostering growth and professional development. Providing training and development opportunities to enhance the MDTs skills and confidence, aligned with best practices. Leading or contributing to training sessions promoting excellence in clinical practice. Acting as a clinical role model and leader, championing evidence-based practice and continuous improvement. Supporting PCN workstreams and strategy development, contributing your expertise to wider objectives where appropriate. Building partnerships with key stakeholders, both locally and nationally, to enhance care pathways and develop holistic care models. Creating and implementing policies, procedures, and protocols aimed at improving patient identification, assessment, and ongoing support.
