Summary
KEY DUTIES To collect and analyse patient information (e.g. medical, biochemical, nutritional, social, cultural, financial and personal) to devise appropriate evidence based nutritional care plan as part of the treatment of their disease(s), adapting advice to ensure that it is practical and realistic for the individual. To calculate individual requirements for nutritionally at risk and compromised patients and advise on dietary manipulation and/or the prescription of nutritional supplements/feeds. To provide and receive complex information which requires empathy and reassurance. Communicating where there may be barriers to understanding and where skills such as motivational interviewing and counselling are required. To contribute to the production and evaluation of resources produced by the department e.g. diet sheets, information and training packs, advice leaflets etc. To ensure that any equipment, furniture or building in need of repair is reported to the correct authority as necessary. To work autonomously but recognise own limitations and when to escalate issue or refer to more senior therapist for advice and support. CLINICAL & PROFESSIONAL RESPONSIBILITIES To ensure effective internal and external 2-way communication with patients, carers and all other Health and Social Care Professionals to negotiate, review and adapt care plans and in all aspects of patient care. To provide a dietetic service to designated in-patient wards, including patients with terminal illness and ensure that patients are seen in a timely manner. To contribute to the development of departmental / organisational procedures and guidelines. To ensure advice offered to patients is based on the best available evidence based clinical practice, under the guidance of Senior Dietitians. ADMINISTRATIVE RESPONSIBILITIES To process referrals for equipment as appropriate to the clinical area, including stock maintenance and collection/cleaning of equipment. To develop and maintain accessible patient information, equipment and resources to complement assessment and treatment and contribute to the patients rehabilitation. To be responsible for the timely and accurate collection of such statistics/records as are required nationally and departmentally and to ensure that confidentiality is maintained. TEACHING & TRAINING RESPONSIBILITIES To participate in the delivery of A placement student training programme and work experience programmes. As a clinical teacher participate in the delivery of the student training programme and assessment of C placement students. Deliver tutorials in the areas of clinical dietetics and professional practice. To develop knowledge and skills in specific therapy practice through a competency-based programme led by the team leader. To contribute to the Induction, development and training of other more junior staff, students and other colleagues as appropriate and as advised by the clinical supervisor. To participate in, in service training, share information from training attended and keep up to date with relevant practice and maintaining CPD portfolio. To identify own personal development needs with supervisor through supervision and Personal Development Plans. To actively contribute to Trust/Departmental activities in relation to Clinical Governance and audit in service area as requested by clinical supervisor LINE MANAGEMENT/SUPERVISORY RESPONSIBILITIES To provide cover for leave and sickness to the clinical service as required. Liaise with senior dietitians and refer to other health professionals to ensure appropriate care pathways for those patients seeking information outside your area of skills and knowledge. To liaise with catering staff and advise on the provision of therapeutic diets. To be a member of the multi-disciplinary team and actively participate in M.D.T. meetings.
