As a Positive Transitions Officer, we are looking for a motivated, engaging and dynamic team-player who shares our passion, enthusiasm and commitment to enhancing the life opportunities of vulnerable young people here in Blackpool.
The role of PT Officer is to support a caseload of young people leaving care who have been referred from Blackpool Council Children’s Services to assist them in making a positive transition from being in care to establishing themselves in their new homes successfully.

It is recognised that good quality housing underpins success in all areas of a young person’s life so you will ensure this transition is carried out in partnership with the young people and that their personal experience is positive, supportive, truthful and empowering.

PT Officers encourage, motivate and working alongside the young people, to provide a range of support that will inspire them to make essential changes for themselves, through clear goal setting and supporting the young people in developing links with statutory and community services to provide sustainable long term support. PT Officers develop long-term outcomes, including exit strategies to enable the young people to successfully sustain their tenancies and prosper in the neighbourhood in which they live in.

This is achieved by providing support to develop practical skills to assist in their ability to improve their own situation and enhance their life choices.

Applicants must possess excellent communication skills, as you will be engaging with care leavers to establish clear goals and shape their individual support plans.
In addition, you will have a good understanding of the requirements of safeguarding children, with knowledge of how to make referrals, awareness of professional boundaries and have experience in working with a diverse range of customers and agencies.

Applicants must possess excellent organisation skills, be conscientious and have a well organised approach to work, as you will be required to manage their own case load.

A minimum of 3 GCSE’s (or equivalent) is essential to include Maths and English at Grade C/4 or above. It is also desirable for applicants to hold a full UK driving licence, with access to a vehicle for work purposes to be able to transport customers, if necessary.

Any offer of appointment will be subject to a satisfactory enhanced level check from the Disclosure and Barring Service (DBS).
